The temple carries the name Thirunandhipuravinnagaram ("the sacred city of Nandi") after the legend describing how it came to be associated with Shiva's bull-mount Nandi. Thirumangai Alvar is understood to have sung in praise of the deity here, and the Nandi association runs through nearly every element of the temple's sacred geography: its theertham is Nandi Theertham, and the surrounding place-name Nandipuram itself commemorates the legend.

Temple tradition holds that Nandi, the sacred bull and vehicle of Shiva, once received relief from a curse by worshipping Vishnu at this spot, giving the town its name Nandipuram, later softened in everyday usage to Nathan Kovil. Tradition also holds that the legendary king Sibi, known elsewhere in Hindu tradition for his extreme devotion to protecting those who sought his refuge, worshipped Vishnu here.

The temple is built in Dravidian style and is understood to date to the Chola period, consistent with its glorification in the Naalayira Divya Prabandham by Alvars active between the 6th and 9th centuries CE.

The temple has a three-tier rajagopuram at its entrance. Within the main sanctum, the presiding deity is flanked by Sridevi and Bhoodevi, the two consort forms of Lakshmi, while a separate metal festival image represents Jagannatha for use in processions, a fairly elaborate arrangement for a temple of this size.

As with most of the smaller Divya Desams in the Kumbakonam belt, the temple's main observance is the shared Vaikunta Ekadashi season in the Tamil month of Margazhi (December–January), alongside its own periodic Brahmotsavam.

Daily worship follows the customary schedule observed at the smaller Chola Nadu Divya Desams.
